To qualify for this program, you must meet the following criteria:

Reside in any of the following Texas counties:
- Tarrant, Dallas, Hood, Johnson, Parker, Somervell, Wise, Collin, Denton, Ellis, Hunt, Hill, Bosque, Kaufman, Rockwall, Hays, Travis and Williamson
Be diagnosed with Endometriosis, or suspect Endometriosis.
Be 18 years or older.

By applying to Endo Forward Foundation's Care Buddy Program, you acknowledge and agree to the following terms and conditions: Endo Forward Foundation acts as a third party facilitating connections within the endometriosis community through the Care Buddy Program. Applicants who have not previously met with EFF are required to participate in a video call with an EFF board member to verify the authenticity of their application as a real person. It is important to note that Endo Forward Foundation does not conduct background checks, and participation in the program is entirely voluntary and undertaken at your own risk. You confirm that you are 18 years of age or older. Participants agree not to hold Endo Forward Foundation liable for any issues arising from their participation in the Care Buddy Program. Endo Forward Foundation reserves the right to modify or terminate the program at any time, with or without notice. Please initial if you agree to these terms.
